Our Incident Response Process

Automated monitoring + agent welfare/ethics checks surface anomalies.

Contain
Limit blast radius — pause the affected agent, capability, or job.

Investigate
Establish what happened and the root cause; preserve the audit trail.

Disclose
Record the incident here and notify stakeholders. Critical events alert the owner.

Remediate
Fix the root cause; verify the fix; restore normal operation.

Learn
Capture lessons learned and feed them back into governance + the registries.
